Please complete this form completely and accurately so that we can get to know you and your pet(s) before your visit. If this is an emergency or your pet requires immediate attention, please call us at 860-399-6249 for a faster response.

Doggy Daycare provided by Animal Hospital of Old Saybrook, its owner, employee(s), representative(s), or any other person(s) affiliated with the company shall hereinafter be referred to as “Doggy Daycare.”

Although "Doggy Daycare" screens all dogs for temperament, watches the dogs carefully, and does not take aggressive dogs, accidents can happen. They can get rambunctious at times, and we cannot be held responsible for injuries and/or death that may occur during their time with us, including the transporting of animals.
In my absence, if my dog should become injured or in need of veterinary care, "Doggy Daycare" has my permission to
  • Consult with my veterinarian. If my dog should require immediate care, I hereby give "Doggy Daycare" permission to bring my dog to Animal Hospital of Old Saybrook or the nearest emergency facility if needed. I understand that I am responsible for all veterinary costs, including the transportation of my dog to and from the veterinarian.
  • I understand that my dog will be taken outside on a daily basis and in play groups with other dogs inside and outside at "Doggy Daycare".
  • I hereby declare that my dog has never shown aggression, bitten, injured, or killed another dog or person. If my dog does, I agree to pay all bills associated with the incident.
  • I hereby declare that my animal has not been exposed to any communicable diseases within the last 30 days and is fully vaccinated in accordance with the "Doggy Daycare" policy
